LiveWell Dementia Hub
The LiveWell Dementia Hub is a community-based service that provides dementia information, support and training for anyone living in Stockton-on-Tees.
The Hub environment has been designed to better support people with dementia and offers a calm space where you can access:
- up-to-date information about dementia
- information about the services, support groups and social activities available locally
- memory service appointments
- dementia training
This includes people with dementia, their families, their carers and anyone who would like to learn more about the condition.
Dementia Advisor Service
We can provide specialist information and advice about dementia, local support services, planning for the future and strategies for dealing with common challenges.
Advisors work with people from the early stages of diagnosis. They make sure people with dementia and their families have the knowledge, skills and confidence needed to help with health, care and self-management.
Key areas of advice are on:
- understanding dementia
- getting a diagnosis and what to expect
- where to access information when needed locally
- accessing social opportunities and peer support networks
- accessing benefits and other entitlements
- carers support
- maintaining your emotional wellbeing
- staying safe at home with assistive technology
- planning for the future
- managing dementia related changes in behaviour
- managing dementia related changes in communication
- managing daily living activities
- maximising health
- accessing health and social services
- care homes
- getting involved in dementia related research
