Multi-Disciplinary Service
A Health and Social Care Team has been developed to provide short term early intervention and prevention to help you remain safely in your home.
The Multi-Disciplinary Service (MDS) is provided by Stockton-on-Tees Borough Council and North Tees and Hartlepool NHS Foundation Trusts. The longer term goal is to prevent avoidable hospital admissions and referral into long term social care.
The service is available to people who:
- are aged 65 years and over
- live in the Borough of Stockton-on-Tees
- are not currently under the care of a social care professional
Those benefiting from the service will receive a wellbeing assessment, and support plan will be developed with a wellbeing facilitator. The support plan may include a referral to other health, social care and voluntary sector services.
You do not need a Care Act Assessment and any support or intervention given during the first 6 weeks is free.

Falls Service
The falls service is a part of the Multi-Disciplinary Service (MDS). The aim of the service is to provide education, assessment and early intervention to help prevent people from falling.
To qualify for a referral clients must be aged 65 and over, live within Stockton-on-Tees and one of the following must apply:
- the person has had more than 1 fall in a 12 month period
- the person has needed medical advice following a fall
